There are many places you can get vaccinated—listed below are CCDPH Partners offering the vaccine. Click here for mobile and pop-up vaccinations. If you are eligible, you can receive an in-home COVID-19 vaccination (residents are eligible if they are unable to leave home due to age, disability, or medical impairment.).
Need a FREE ride to get your vaccine?
Cook County Department of Public Health is partnering with Kaizen Health to offer FREE rides to get people to and from their vaccinations. Schedule your ride by calling 833-308-1988. Kaizen can provide vehicles to accommodate most residents, including those in wheelchairs or those who require child seats. This service is provided at no cost to residents.
You can get vaccinated if you’re homebound.
Cook County Department of Public Health COVID-19 Vaccine Provider Partners are providing in-home COVID-19 vaccinations for suburban Cook County residents who are unable to leave home due to age, disability, or medical impairment.

After sign-up, you will receive a confirmation email or phone call from one of CCDPH’s COVID-19 Vaccine Provider Partners to schedule your in-home vaccination when the vaccine is available to you. Please be patient as we respond to a large number of requests.
