Thank you for your interest in having the Cook County Department of Public Health (CCDPH) participate in your upcoming event in suburban Cook County.  If you are requesting vaccinations/immunizations only, please use this link.

Who We Serve

CCDPH serves municipalities within suburban Cook County with the exception of the municipalities that have an independent state-certified local health department (Chicago, Oak Park, Evanston, Stickney and Skokie).

Before You Submit Your Request

• Please submit your request 3–6 weeks prior to your event.
• You can expect a response within 5–7 business days.
• Submission of this form does not guarantee participation.
• Due to limited capacity, your request may be referred to a partner organization.
