The Cook County Department of Public Health issues permits and licenses for several environmental health programs in suburban Cook County. Click on the program below to download the corresponding application.
Mail completed applications and applicable payment to the address listed on the application. Make checks or money orders payable to Cook County Department of Public Health. No cash payments accepted. Fees include costs for licenses, permits, plan reviews and inspections.
Mobile Home Parks
Application to Operate a Mobile Home Park
Submit the above application and appropriate fee for the following:

| Annual License to Operate a Mobile Home Park mobile home space | 100.00, plus $12.00 per mobile home space |
Residential Swimming Pools and Spas
Unincorporated Cook County residential swimming facilities:

| Residential Above Ground Swimming Pool Permit Application | 75.00 |
| Residential Spa Permit Application | 75.00 |
To apply for a construction permit for a public swimming facility, pre-qualified architects, engineers and contractors must complete the IDPH permit application . For more information, please contact 217-782-5830.
