
WePlan 2020
In December, 2016, the Cook County Board of Commissioners approved WePlan 2020, CCDPH’s Community Health Assessment and Improvement Plan, conducted every five years with community partners and residents to develop and implement a strategic health improvement plan.
This was the culmination of months of work by numerous staff and involved key stakeholders from across suburban Cook County. Data from four community-level assessments (the Community Themes and Strengths Assessment (CTSA), Forces of Change Assessment (FOCA), Community Health Status Assessment (CHSA) and Local Public Health System Assessment (LPHSA) were analyzed and used to determine three community health priorities, including:
Health Equity
To reduce structural racism, a root cause of health inequities, and advocate for pro-equity policies.
Chronic Disease
To reduce inequities and the burden of chronic disease by cultivating environments, healthcare systems and a culture that promotes health.
