CCDPH Local Public Health System Assessment (LPHSA) Retreat

We cordially invite you to participate in the CCDPH Local Public Health System Assessment (LPHSA) Retreat, an appraisal of our local public health system. The public health system includes all public, private and voluntary entities that contribute to the public’s health and well-being in the Cook County community. The meeting will be conducted as a day-long retreat on September, 1, 2015 at the Oak Forest Health Center Campus in the auditorium and facilitated by the Illinois Public Health Institute. The retreat will start at 8:00 am for registration and light breakfast and end at 4:00 pm. Lunch will also be provided.
